Medical
Your medical card can be requested by contacting the Member Services. The Member Service phone numbers are listed on page 30 of the Benefit Guide.
Dental
Your dental card can be requested 24/7 at https://accounts.principal.com/app/bookmark/0oadm2qe1orihoKba5d7/login. You would just need to make a Principal Account. You can also find in-network providers at https://www.principal.com/find-dentist using your zip code. Group Number is 1004699-10001
Vision
VSP does not provide insurance cards as coverage is verified using your personal information (SSN, DOB, Name) at the provider. You can find in-network providers at: https://www.vsp.com/eye-doctor
